What is a 30 second shot clock violation. Does the shot need to be out of the shooters hand or hit the rim before 30 seconds?

Out of the hand just like a buzzer beater at the end of the game.

Out of hand. Just like end of either half or overtime. But if shot is airball (doesn't score or at least draw iron) then it's a shot clock violation when clock expires.

Seems to work against certain teams. Maybe not against up-tempo, fast-break type who having good 3 pt shooters.
