How did this become a phrase to replace why? Is it a sign of low intelligence or white trashiness?

"How did it come to pass that such a query should grace your mind?" Became "How come you ask?" That's just efficient, not trashy.

My dad is 70 and often says "how's that?" when he wants someone to repeat something. I've heard many people from his generation say the same thing. Sounds strange.

Oh well, I'm sure when I'm his age my boys will say the same about things I say.
