Just for S&Gs I looked over the rest of the incidences discussed in the link above:
Boise State:
Tennis program provided impermissible benefits directly to players, let an athlete play beyond eligibility, let international athletes in before they qualified, head coach never reported violations. Also, recruiting violations in track and football.
Baruch College:
Players paid for work they didn't do, got free lodging from a coach.
IUPUI:
Players who weren't academically eligible allowed to play.
Kean University:
Bball coach involved in changing a players grade to make them eligible. School paid some of the cost of travel to spain for athletes, including cash payments. Not reported by coach.
I think it is pretty clear to see what these "Lack of institutional control" charges have in common.
