What's funny to me is that as an employer, you could make a sign like that with 2 or 3 employees names on it and it would be plausible. Year in and year out, about 75% of the safety issues are associated with the same people.

Despite training, etc, some people are just accident prone...at work and in their personal lives too.

The "Joe" at my last large employer was named "Zack". And they didn't fire him until the second time he lit Li-ion batteries on fire (or dunked in water), resulting in a building evacuation from the fumes.
