If you delve a little further into the hormones/endocrine stuff, your second question actually becomes pretty ridiculous — and also calls into question your apparently rigid interpretation of "evolutionary biology".

I run into women that were clearly born men all the time. And I just treat them as a regular human being. Done, and everyone moves on with life.
